Our CCG was created on 1 April 2020. Before this date we existed as three separate organisations, NHS Airedale, Wharfedale and Craven CCG, NHS Bradford City CCG and NHS Bradford Districts CCG, who worked closely together.

Case studies
The Bradford Healthy Hearts programme was setup in 2014 by NHS Bradford Districts CCG with the aim of reducing the number of deaths caused by heart attack and stroke in Bradford and district. At the time, Bradford had one of the worst death rates from heart disease in England.
The campaign which was implemented over three years, targeted three main areas:
- vascular disease – including patients with high blood pressure, high cholesterol, diabetes and peripheral arterial disease
- atrial fibrillation (AF) – a condition which triggers abnormalities in the rhythm of the heart
- heart failure.
In its first year Bradford’s Healthy Hearts had a dramatic improvement in the prevention of heart attack and strokes among local patients. Over 10,000 patients in the Bradford area were helped by the campaign, and it already potentially prevented 50 heart attacks and 50 strokes. More than 800 people were placed on vital stroke preventative medicine, which reduced the risk of stroke by up to 75% in these patients, avoiding nearly 50 devastating strokes per year.
The risk of stroke for people with atrial fibrillation (AF) – a heart rhythm disorder – can be reduced by more than two-thirds by taking an anticoagulant medication – such as warfarin – prescribed by a doctor. In the first phase of the campaign, GPs also reviewed patients taking cholesterol-lowering statins to ensure that they were receiving the best care available to reduce their risk of a future stroke and heart attack.
By switching to different statins, over 5,000 patients have reduced their cholesterol level, and over 4,000 patients at moderate to high risk of heart attack and stroke have been prescribed statins to reduce their risk.
The positive outcomes from Bradford’s Healthy Hearts, include the reduction of unplanned admissions to hospital for stroke and heart attack by 10%.
The Bradford’s Healthy Hearts campaign has received multiple national awards for its work, including from the HSJ, BMJ and at the General Practice Awards.
In April 2019, the Bradford’s Healthy Hearts programme was adopted by the West Yorkshire and Harrogate Health and Care Partnership. The West Yorkshire and Harrogate Healthy Hearts programme – which builds on the success of the award-winning Bradford’s Healthy Hearts – aims to help reduce the impact of heart disease and prevent the number of heart-related illnesses, including heart attacks, stroke and diabetes.
The programme, created by the West Yorkshire and Harrogate Health Care Partnership, aims to reduce the number of people affected by heart disease by 10% by 2021, meaning 1,100 fewer cardiovascular disease (CVD) incidents by 2021.
